There is a phishing site impersonating other websites at survey-eu1.hsforms.com. This site has been reported 16 times as a scam. The site tries to trick people into giving out personal information by pretending to be something it is not.

The warning signs of this scam include a website that looks similar to a real one but has a different web address. If you get an email or message with a link to this site do not click on it. The site may also ask for personal information like passwords or credit card numbers.
